Sounds like a homemade boot I made many years ago. I´ll check if the CDs are still working. I´m not very optimistic about still have the original files for the art, though.

Here it is. Thankfully the CD seems to be working and I had an organized artwork database. Don´t remember why there´two version of the back cover though.

Please note this is a very early 2000s homemade CD. I didn´t have any remastering resources back then; just straightforward transfer from analogic source. I actually wouldn´t know how to do much better now.

Anyway, I´ll be really glad if this is what you´re looking for.
